Short Term Accommodation or Respite
When you feel tired of the same routine, have just been discharged from hospital, or the people who love and care for you need some time to rest and recuperate, why not come to us to explore respite options. We have a comfortable home waiting for you, and you will be cared for and have an opportunity to meet new people.
Medium Term Accommodation
Are you are waiting for your home modifications to be ready, assistive technology to arrive or your confirmed Specialist Disability Accommodation option to be finalised? We have a Medium-Term Accommodation home with your name on it as you wait for your permanent housing solution. Did you know that NDIS can fund you up to 90 days if you are eligible? Our accommodation is flexible should you require more time to stay.
Supported Independent Living
At Centred Care, your needs are assessed fully and options to help you live the life you want are explored with you at the centre. We offer a range of living options tailored to your needs and interests. Each home is set up to suit the needs and personalities of the people who live there. We support people with a variety of needs, ranging from medium to complex needs. Our experienced staff will cater to all support needs and they pay particular attention to your wishes, goals and hopes.
Independent Living Options
Centred Care offers alternative ways of living arrangements to our participants. The choice is yours when it comes to with Independent Living Options. Participants can share a home with friends or family. Or they may choose to live in a shared arrangement or on their own with tailored support. We tailor support to suits whatever you need.
Our homes are staffed up to 24 hours, 7 days, and our team will provide every participant with the necessary supports to thrive.
Assistance with Daily Tasks and Personal Activities
We want you to feel comfortable and confident doing everyday activities and we understand the value of having a comfortable clean environment.
We have a team that is ready to assist you to meet your goals, take part in daily activities, be out and about in the community enjoying everyday things, help you develop social skills and make friends, stay safe and healthy, improve your independence at home, or assist you with staying fit and strong.
We are here for all your needs, such as personal care, cooking, cleaning, grooming and whatever activities you need assistance with to increase your independence. It may be that you need help with seating and positioning, mobility, and other personal needs, we are here for you.
Therapeutic Supports
We have a team of dedicated social workers with a vast knowledge and experience in child development, trauma, and family systems. We are a trauma informed organisation, and we are committed to attend to all your therapeutic care needs.
Our social workers offer case management support, therapeutic support, review support plans and make recommendations for the best possible model of care to ensure your needs are at the core of our service delivery.
At Centred Care, we understand that life is not linear. Our therapeutic services are tailored to each individual and they are reviewed and adjusted in line with your changing needs.
Travel and Transport
We make sure that you get to the places you need to be in the safest and most comfortable way with people who understand and are trained to meet your needs.
Everyone deserves to get out and about in the community with ease. Do you need to be picked up from hospital, go see a doctor, visit a friend, go watch a movie or go to your favourite restaurant? We are here to make It happen.
We have a variety of transport options and certainly one to suit your needs whether you are a wheelchair user, need assistance with mobility or have stiff joints we can get you to where you need to be safely and comfortably.
We have a variety of vehicles and experienced carers to suit all needs.
Community Participation
