Centred Care is a Perth-based, NDIS-registered provider. We support people with disabilities through a range of person-centred NDIS services in the Perth Metro, WA area. Servicing all ages, including children with complex needs or those in ‘out of home’ care.
Thembe and Sipho Moyo founded Centred Care in 2022, motivated by their own personal journey in navigating the NDIS for their autistic son. Our Director Thembe is a dedicated Social Worker, who has a deep commitment to supporting vulnerable and at-risk individuals. Driven by her personal experience and passion, Thembe ensures that Centred Care leads the way in providing exceptional, personalised supports to people with disabilities and children in out of home care.

We provide SIL services to NDIS Participants living with Psychosocial, Neurodevelopmental and other disabilities. Our 24/7 supports are designed to help increase your capacity in your everyday independence. You can enjoy these services within one of our welcoming properties, or from the comfort of your own home.
We offer STA services to NDIS Participants in a warm, safe and supportive environment. Respite is designed to offer you a break from your usual home environment, in times where you may need it most.
We understand the importance of engaging in meaningful social and community participation activities. Our support workers are here to guide you every step of the way, making sure you feel confident and supported.
We help you to understand your NDIS Plan and to connect with the right service providers to support you with your goals. We’ll identify your strengths and use a collaborative approach to break down barriers and aim
We provide supports to help you maintain your independence in your home environment. These services are designed to help you build capacity through assistance, supervision or prompting with your daily personal activities.
We welcome you to join us for our monthly adult activities. Our fun experiences are designed to foster social interaction in a supportive environment.
We provide both In-Home Care and Day Respite services to foster carers or parents caring for children with complex and challenging behaviours due to trauma or disabilities. These services are designed to empower carers and preserve family bonds and can be funded through the Department of Communities or your NDIS Core Funds.
I can’t thank Centred Care enough for the level of commitment, professionalism, care and nurturance they have provided to our young person for over a year now.
