Supported Independent Living (SIL)
Centred Care offers Supported Independent Living (SIL) services, tailored to NDIS Participants living with Psychosocial or Neurodevelopmental Disabilities. We are dedicated to providing high-quality, personalised supports that align with your individual needs and NDIS goals. Our 24/7 supports are available at various ratios, from 2:1 to 1:3.
Short Term Accommodation (STA)
At Centred Care, we offer Short Term Accommodation (STA) to NDIS Participants in a warm, safe and supportive environment. Whether you need a break from your usual routine, or require additional supports, our respite services are designed to align with your NDIS goals. Short Term Accommodation (STA) is typically funded for up to 14 days at a time. Some NDIS participants may be able to access up to 28 days.
Assistance with Social, Economic and Community Participation
Our dedicated team of support workers understand how important it is to engage in meaningful social and community activities. Whether you’re looking to expand your social network, start volunteering or explore new hobbies, we are here to support you in breaking down barriers and achieving your goals.
Support Coordination and Psychosocial Recovery Coaching
Our experienced team of Support Coordinators and Psychosocial Recovery Coaches provide supports to NDIS Participants living South of the River, Perth WA.
Assistance with Daily Personal Activities
At Centred Care, we provide supports to NDIS participants needing extra help to maintain their independence at home. Our experienced support workers will assist you with self-care activities and personal domestic activities. These services are designed to help you build capacity with daily personal activities, through assistance, supervision or prompting.
Group and Centre Based Activities
Centred Care welcomes NDIS Participants to our monthly Group and Centre Based Activities. This program is designed to foster social interaction and fun, in a safe and supportive environment.
In-Home Care and Day Respite
At Centred Care, we understand the unique challenges faced by foster carers and parents who care for children with complex and challenging behaviours due to trauma or disabilities. Our qualified and experienced team of carers provide both In-Home or Day Respite care supports.
